Fill out a separate form for each Youth in your party.
Youth 17 and under are welcome to attend free of charge for entry, but you still need to fill out this form for each attending youth so we know what optional items may be needed for each. For instance you may want to bring them to the Friday night banquet with you. In that case the meal and award ceremony does have a fee and meal options need to be selected.
